Vaginal penetration isn't possible because of the male anatomy and it will take lots of reconstructive surgery to achieve a normal size vagina and it won't be pleasurable for obvious reasons therefore anal intercourse is usually preferred by such patients. Her voice will remain feminine as long as she remains on hormone drugs given to her by her doctor and it has the side effects of female hormone related cancers.
