Well @kolaboofreally had a go @ this yesterday. Cutting the genitalia (which is female circumcision/mutilation) on this cake is very insensitive and inappropriate. There are a million other ways of being artistic, why choose a black woman, a caricature for that matter.

For your info, a cake maker NEVER unilaterally decides what design to use in making a cake, they are usually given specifications on what to make so the excuse that the baker is black is unfounded.
