I was 34 weeks pregnant when I used British Airways to go to the US. Once your are not up to 36 weeks and you have your fit to fly letter from your Doctor, you are good to go. As for entering the US, I was well covered. Remember most blacks look fat so I guess they assume it's one of those things.
Many still do not cover but once you can establish that you can afford your bills e.g 10k dollars and credit card, you are allowed to pass.
Travel history also helps.The immigration officers aint that strict. They can fish out likely culprits. they are trained to do that.
Actually if you declare at the US embassy that you're going to give birth and show proof of payment no one will turn you back with a big belly.
